A temporary employee was stacking wooden trusses about 3 feet high. When he was finished, a forklift lifted up the trusses to be moved elsewhere. The trusses on the forks shifted and the employee s gloved, right ring fingertip got caught between the bottom truss and the forks. He sustained the amputation of his right ring fingertip without bone loss.

An employee was on top of a press machine when his foot slipped into the press, becoming trapped. The press came down on his foot, breaking toes and amputating three toes.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

An employee was working on a conveyor belt in the chipper room that was not functioning. While inspecting the uncovered gearbox, the conveyor system started. The employee sustained an amputation to their right little finger. The conveyor system was not locked out at the time.

An employee was placing trusses on conveyor rollers to be stacked. The rollers could not roll the trusses, so the employee pulled the trusses onto the rollers. One of the spinning rollers caught their hand, resulting in three fingertip amputations.

An employee reached down to pick up a box next to a roller conveyor. His gloved left hand was caught in the chain and pulled into the sprocket. Three fingers were injured: the middle and index fingers were cut to the bone, and the ring finger was amputated at the nail.
