An employee was assisting in the removal of a trench box from an excavation. The employee noted that a rigging hook was stuck on a corner of the trench box and went to unlatch it. At the same time, the excavator operator lifted the rigging and the employee's middle finger was caught between the rigging and the trench box resulting in a fingertip amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
