An employee was sweeping grain and got caught in the drag chain conveyor after stepping into an opening in the walking surface. The employee suffered the surgical amputation of three toes on the left foot.
On October 29, 2016, an employee went to the top of the bin and opened the manhole hatch to repair the sweep auger. The employee was later discovered at the bottom of the bin approximately 80 feet below the opening. The employee suffered a broken left leg and fractured vertebrae.
