On October 29, 2016, an employee went to the top of the bin and opened the manhole hatch to repair the sweep auger. The employee was later discovered at the bottom of the bin approximately 80 feet below the opening. The employee suffered a broken left leg and fractured vertebrae.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.
