An employee was palletizing finish good products. When he turned towards the pallet while holding a package, he tripped on the corner of the pallet and fell to the floor resulting in fractured ribs.

An employee was pushing containers off of a machine's conveyor belt. Her right hand was on the containers that were accumulated on the belt when her right pinky finger was caught between the machine's product lane straightener mechanism, resulting in a fingertip amputation.
