On May 22, 2019, at 12:00 PM, while an employee was trying to remove a valve on a forklift truck tire, it exploded in his face causing two lacerations on the face and fractured left elbow. The tire was not attached to the forklift at the time.
An employee was using an air compressor on a multi-wheel tire when the flange and the tire burst, causing the wheel's lock ring to strike the employee in the face. He sustained a laceration and facial fractures.
