An employee fell from the back of a garbage truck to the ground near the road. The employee was hospitalized for a fractured skull and a back strain. The employee also had a seizure and/or a brain bleed.

On March 11, 2021, a temporary employee was on a route picking up garbage. While he was cycling the hopper, a metal pole sprung out and struck him in the face, resulting in facial fractures that required hospitalization.

An employee returned to the scale house after dumping at the landfill. He parked his vehicle on the ramp. As he stepped out of the truck, he missed the ramp and fell onto the concrete sidewalk. His left hip fractured.

A temporary employee had been picking up trash cans and dumping them in the rear loader of a garbage truck. At the time of the incident, he was helping the truck driver back up in the street. As the driver was backing up, the employee's right hand (finger) was caught between the rear loader and a 4 x 4 piece of wood, resulting in a finger amputation.

A maintenance employee was moving a forklift from the loading dock when the forklift fell off the dock. The employee was struck by the overhead portion of the roll bar, injuring his right knee. He was hospitalized and underwent surgery.

An employee was standing on the driver side rear step of a trash truck while traveling down a road to collect residential trash. The employee went to signal the driver and slipped off the step while the truck was traveling 8-9 miles per hour. He fell 18 inches to the ground and was hospitalized with a fractured ankle that required surgery.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
