On September 15, 2021, an employee was trying to collect machined dust from the auger area of a machine. While in the small trench containing the auger, the employee slipped. His right hand was caught in the auger and severely injured. All five fingers were medically amputated. The machine was not locked out/tagged out at the time.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was responding to a maintenance call about a snake outside a building. As the employee tried to kill the rattlesnake, it bit the employee twice. The employee was hospitalized.

An employee was in the warehouse helping to troubleshoot a bagging machine. The machine's anvil activated and the employee's left middle finger was crushed by the bottom sealing bar. The fingertip was medically amputated at the last joint.

An employee was accessing a rotary airlock to clean it out when their fingers were caught by the star valve as it was rotating. The employee sustained amputations of their left middle and ring fingers.

An employee was setting up for operations on a centerless plunge grinder. The machine's cutting wheel lacerated the top of the employee's right hand. The employee was hospitalized.

An employee was loading a machine with powder when the machine activated, the punch came down, and their right hand was caught. The employee's fingers were amputated.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
