An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.

An employee went to step off material handling equipment to grab their jacket when the throttle of the vehicle engaged. The vehicle reversed and the employee's left foot was crushed between two material handlers, resulting in a fracture.

An employee was working at a height of approximately 20 feet and was standing outside his forklift's basket attachment. His forklift was struck by another forklift, causing the employee to fall to the warehouse floor below. The employee sustained facial injuries, broke two ribs and an arm, and also suffered a head injury. The employee was wearing a harness at the time but it was not connected to an anchor point.

The employee stopped the four wheeler near the administrative office to allow a log truck to exit the property. The log truck backed up, and the employee's left ankle was caught between the log truck trailer and the four wheeler, resulting in a fracture and laceration.

An semi-truck was backing into a loading dock when it struck an employee who was walking on the bay floor. The trailer pinned the employee against the dock bumper. The employee suffered crushing injuries and was hospitalized.

Employees were unloading a box truck using a stand-up lift with pallet fork extenders. As the stand-up lift traveled up an incline, it struck a parked sit-down lift and the injured employee's left ankle was pinched between the lifts, resulting in lacerations and an ankle fracture. The employee was hospitalized.

An employee had just replaced a belt on a silo blower. While the employee was testing the blower by manually turning the belt, the belt and pulley caught the employee's fingertip. The fingertip was amputated.

An employee was setting up the coding station on a piece of equipment when they took a step back and tripped on a guard that was on the floor. The employee fell onto one leg, resulting in a left hip fracture. The employee was hospitalized and required surgery.
