At about 10:00 a.m. on July 13, 2025, an employee was cleaning the adhesive from running rollers using a rag soaked in cleaning solvent to prevent contamination. The rag was caught between two rollers and pulled the employee's right hand between the rollers. The employee sustained right hand crushing injuries, de-gloving injuries (palmer and dorsal sides), and fractures. The employee was hospitalized and required surgery. The machine was not locked out/tagged out at the time.

On May 8, 2025, an employee developed a cough while cleaning up chemical sewage from the floor. He was hospitalized, having suffered an allergic reaction to chemical product vapors.

An employee was walking between a stack of pallets and a rack when they tripped over a pallet and fell to the floor. The employee sustained a left hip fracture.

An employee was removing a carbon dioxide line when red liquid was found on the ground. The employee reported a "red spot" on their left wrist, inside elbow, and chest after washing down the spill. The employee experiencing escalation of symptoms, including a rash/burn on their arms and respiratory symptoms after the shift. The employee was hospitalized with chemical pneumonitis, chemical dermatitis, and chemical asthma exacerbation.

Two employees were lowering a large roll using a hoist. One employee's hand was caught between the roll's steel core and a storage rack, causing a thumb injury with exposed tissue.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.

An semi-truck was backing into a loading dock when it struck an employee who was walking on the bay floor. The trailer pinned the employee against the dock bumper. The employee suffered crushing injuries and was hospitalized.

Employees were unloading a box truck using a stand-up lift with pallet fork extenders. As the stand-up lift traveled up an incline, it struck a parked sit-down lift and the injured employee's left ankle was pinched between the lifts, resulting in lacerations and an ankle fracture. The employee was hospitalized.

An employee had just replaced a belt on a silo blower. While the employee was testing the blower by manually turning the belt, the belt and pulley caught the employee's fingertip. The fingertip was amputated.

An employee was unloading a truck and sustained frostbite to his left index finger due to cold temperatures.

An employee was operating a forklift in a warehouse. The employee was turning the forklift to go down a ramp to the dumpster when the brakes stopped working and the forklift went backward out the dock door. It landed on its backside on the ground and pinned the operator's right arm underneath. The employee sustained a fractured lower arm and wrist.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
