Final narrative
An employee was removing a roller from a conveyor belt. The roller fell, smashing his left little finger against the floor and amputating the tip.
An employee was changing a belt on a blower motor. The machinery started and the pulley pulled in the employee's right glove. The employee was injured and underwent a surgical amputation of the right ring finger.
While attending the slab line, an employee observed that dough was accumulating on the plastic blades. He proceeded to clean the blades. His left index finger came into contact with an adjacent blade that moves laterally (left to right), resulting in a partial amputation.
