An employee was loading and unloading trailers on a dock when he was involved in an altercation with a coworker. The coworker hit him on the left side of his face with a dock hook. He was knocked unconscious and hit his head on concrete when he fell, suffering facial fractures and a concussion.

An employee was attempting to lower a dock plate. He lifted the dock plate up, then placed his fingers between the dock plates in order to fold the plates out so he could lay them flat. In doing so, his right hand fingers were pinched between the dock plates. His fourth finger needed stitches and his third finger was cut to the bone. One of his fingers required partial surgical amputation.

Family Dollar

A store employee was pushed by a shoplifter into the frame of the entrance door to the store. The employee's head struck the doorframe resulting in head injuries and contusions on her hand and arm. The employee was hospitalized.

A patient reached over the nurse's station, grabbed a metal three-hole paper punch from over the desk, and proceeded to attack the employee with it. The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.
