On June 19, 2019, an employee was operating a machine that carries shingles onto a roof when the belt came off. As he attempted to set the belt onto the machine's pulley, his right index finger was caught between the pulley and the belt, resulting in an amputation of his right index finger without bone loss. The machine was energized at the time of the incident.

On or about August 2, 2016, at approximately 3:30 p.m., an employee was helping to install roofing underlayment when he stepped on wet membrane material and slid off the roof, falling more than 12 feet to the ground below and lacerating his head and tongue.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
