An employee had just finished unloading his truck. He pulled the dock plate up and reached to pull the door down. His foot went between the dock plate and the trailer, causing him to fall to the right and break his leg. He was hospitalized, requiring surgery.

An employee was driving a forklift toward the parking area when he struck another forklift, crushing his left foot.

The injured employee was selecting food product from a pick slot while standing between her cart and the pick slot. Another employee approached on his tow tractor; he went to turn his tow tractor around so the hitch could attach to his cart, which was next to the injured employee's cart. While the tow tractor was turning around, it struck the injured employee's cart. The injured employee was struck by the hitch of her cart and was pinned between her cart and the pick slot. The employee sustained a severe laceration to her left leg.

An employee was loading a truck using a pallet jack. While backing up, the employee's right foot was caught between a pole and the pallet jack. He sustained a crushing injury.

An employee was driving a stand-up forklift while following behind a co-worker who was also driving a stand-up forklift. The co-worker slowed down to stop when the employee ran into their forklift and their left leg contacted the forklift's fork, resulting in a laceration.

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.

An employee went to step off material handling equipment to grab their jacket when the throttle of the vehicle engaged. The vehicle reversed and the employee's left foot was crushed between two material handlers, resulting in a fracture.

An employee was working at a height of approximately 20 feet and was standing outside his forklift's basket attachment. His forklift was struck by another forklift, causing the employee to fall to the warehouse floor below. The employee sustained facial injuries, broke two ribs and an arm, and also suffered a head injury. The employee was wearing a harness at the time but it was not connected to an anchor point.

On November 8, 2023, an employee was operating a stand-up pallet jack in the warehouse. As he was backing up, the pallet jack struck the basket of a parked aerial lift. His right ankle became caught between the pallet jack and the aerial lift basket resulting in a fractured ankle.

On October 26, 2023, at approximately 12:00 PM, an employee operating a four wheeler was on their way to an off-property break location. The employee stopped the four wheeler near the administrative office to allow a log truck to exit the property. The log truck backed up, and the employee's left ankle was caught between the log truck trailer and the four wheeler, resulting in a fracture and laceration.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
