Final narrative
An employee was building pallets using a nail gun. As he bent down to nail a pallet, the gun fired into his leg, lodging a nail into his knee.
An employee was performing quality control on an 8-foot stack of pallets while standing on a 4-foot ladder. The employee was using an air hose to blow dust off of the pallets. The air hose was wrapped around the ladder. When the employee tugged on the air hose, they fell off the ladder onto the ground and fractured their forearm.
An employee was unjamming boards from a rip saw and contacted the spinning blades. He sustained three lacerations to the left hand and forearm, broken fourth and fifth metacarpal bones in the left hand, and a broken ulna in the left forearm. The machine was unguarded at the time.
An employee was performing maintenance work on a wood packaging line to fix a portion of plastic that was stuck when his left middle finger was pinched between the machine basket and a metal bar frame, amputating it above the first knuckle.
On a three head saw machine, an employee was using a hand tool/bar to lift a chain that had a jammed buckle. The employee pressed the foot pedal to move the chain and free the buckle. When the pedal was pressed, the chain moved and caused the hand tool/bar to jerk, which pinched and amputated the employee's right-hand index finger between the chain guard and tool.
An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.
An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.
