An employee was performing quality control on an 8-foot stack of pallets while standing on a 4-foot ladder. The employee was using an air hose to blow dust off of the pallets. The air hose was wrapped around the ladder. When the employee tugged on the air hose, they fell off the ladder onto the ground and fractured their forearm.

When the pedal was pressed, the chain moved and caused the hand tool/bar to jerk, which pinched and amputated the employee's right-hand index finger between the chain guard and tool.
