Final narrative
An employee was waiting out a storm in his work truck but had to use a nearby portable toilet. While he was in the portable toilet, a lightning strike occurred approximately 50 feet away. He collapsed after coming out of the toilet. He suffered a concussion and brain swelling, requiring hospitalization.
While installing a sewer line using the horizontal directional drilling method, an employee was helping to add a drill stem to the tail string when his hand got too close to the nylon sling and the drill pipe and was crushed between the two. His right hand was amputated.
On September 25, 2023, an employee was working in the body department installing doors, hoods, and tailgates. The employee finished their shift and was hospitalized later that evening for a back injury.

The employee sustained electrical burns on his hands extending up to his elbows and a fractured ankle.
An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.
After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
