An employee was removing parts from a CNC machine after the completion of a cycle. Upon machine restart, the employee noticed a T-wrench tool had been left inside the machine. The employee reached to remove the tool when their glove contacted the face mill insert, resulting in a partial amputation to the right index finger.

On February 8, 2022, an employee was clearing a jam on a machine belt. When the equipment began running their finger was partially amputated.

An employee was retrieving a piece of plastic from a plastic cutter machine when the blade amputated his left ring and middle fingertips. The machine was guarded at the time.
