On February 8, 2022, an employee was clearing a jam on a machine belt. When the equipment began running their finger was partially amputated.

An employee was getting parts out of a box to inspect, tripped on the corner of a pallet and fell. The employee was hospitalized with injuries to the left side and right arm.

An employee was retrieving a piece of plastic from a plastic cutter machine when the blade amputated his left ring and middle fingertips. The machine was guarded at the time.

An employee was resetting the chain at the core station. He stepped onto the foot pedal that raises and lowers the hoist as he grabbed the chain and sustained amputation of his left index and ring fingers.

An employee reached into the power transmission equipment to adjust a belt while it was running and suffered a broken arm.

An employee was setting up a print job on a paper converting machine when his left middle fingertip was caught in a nip point on a roller and amputated. The roller was not guarded at the time of the incident.

On May 9, 2025, an employee was using an air hose to clear debris from the courtyard area when she slipped and fell. The employee was hospitalized with closed fractures to her left fibula, tibia, and medial malleolus.

On April 24, 2025, an employee was opening a cylinder door on a hydraulic bridge to remove lumber when their left middle finger was caught between the door and the hydraulic bridge, resulting in an amputation without bone loss.

Two employees were using a plywood chop saw. The injured employee was holding one end of the saw with their right hand when the operator on the other end pressed the start button. The saw blade contacted the injured employee's right hand, resulting in multiple finger amputations when it amputated the hand above the thumb.

On January 17, 2025, at 12:15 p.m., an employee was moving 11-gauge steel sheets from a holding cart using a pendant-controlled, 1-ton jib hoist equipped with a plate clamp lifting attachment. With the plate clamp empty and open, the employee began raising the hoist. The clamp became caught on the bottom of the cart, tipping the cart in the direction of the employee. Several sheets of steel slid off the cart and struck the employee, resulting in several broken ribs, a broken left leg and ankle, a fractured kneecap, and a torn left ACL. The employee was hospitalized.

An employee was operating an air hoist to rotate a blending mill counter-clockwise. After he connected the hook to the mill, the hoist was raised and his hand was pinched between the hook block and the ring on the mill. The employee sustained a partial amputation of his right thumb.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
