An employee was unhooking a cable from a block of metal. The end of the cable was fitted with a quick disconnect device. As the employee removed the cable, the device closed and cut the employee's left middle finger. The employee was hospitalized for infection prevention.

An employee had taken a 10-foot x 1-inch piece of steel rod out of a straightening machine. The piece of steel was buckled in the middle. With his glove on, the employee grabbed the end of the piece of steel. At that time, the buckle released, causing the end of the steel to jump up and hit his hand. He was hospitalized for surgery on a broken middle finger on his left hand.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

During a routine inspection, an employee placed their hand on a pipe to check a weld. The pipe clamp activated, closed on the pipe, and trapped the employee's right middle finger, resulting in a partial amputation and fracture to the fingertip as well as a laceration to the ring finger.
