An employee was bending over to remove a license plate from a car. He then stood up, lost his balance, and fell to the ground. The employee sustained a contusion to the left side of the forehead, a subdural hematoma, and a laceration above the left eye.
An employee was stepping out of his van when one of his cleats got stuck on the running boards, causing him to fall forward to the ground and sustain injuries to his head and foot.
An employee was driving a golf cart in the lot area. The employee was returning inside the building because it started raining. There was an open metal bed trailer parked in the lot with the tongue hitch sticking out. The employee did not see the trailer's tongue hitch and hit it with the golf cart.
