Federal OSHA recorded a severe workplace injury at US ARMY CORPS OF ENGINEERS, SAINT LOUIS DISTRICT, USACE Saint Louis District Service Base, SAINT LOUIS, MISSOURI 63118 on — Amputations, affecting the fingertip(s).

At about 10:20 a.m. on August 28, 2019, a St. Louis Service Base employee was using an industrial shear to cut 6-inch by 6-inch strips of 3/8-inch-thick steel for an ongoing fabrication project. While placing the metal sheet into the shear, the employee closed the clamping apparatus. The apparatus closed on the employee's right ring finger, pinching it under the metal sheet and amputating the last quarter-inch, with exposed nerve endings and bone damage. The fingertip was medically amputated at the first knuckle.

An employee fell while exiting a pickup truck, landing on the asphalt parking lot surface about 22 inches below. The employee suffered multiple fractures to the left femur (including at the neck of the hip socket) and was hospitalized, requiring surgery.

His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

An employee was visiting a patient's home to fit them with a wearable cardiac defibrillator. As he was walking from the front door to his car, a large dog broke free from a chain and attacked him. He was bitten on the forearm and fell. The patient was then able to restrain the dog. The employee was hospitalized with severe dog bites and required surgery.

An employee tripped over a junior I-beam about 4 feet long and 6 inches tall that was installed for circulating water valves and fell to the ground, resulting in a broken right-side rib.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
