An employee was cutting small pieces of wood on a bandsaw. The saw amputated the tip of his left little finger.

An employee was driving a double pallet jack down an aisle when another piece of equipment came through the aisle. Realizing that the two pieces of equipment were about to collide, the employee jumped off the pallet jack. It then ran over his leg, breaking his leg/ankle.

An employee was operating a forklift when the load shifted. The employee jumped from the forklift and it rolled backward, falling off the ramp and pinning the employee underneath it. They sustained head trauma and injuries to the shoulder, arm, pelvis, and leg.

An employee was operating a rider-style powered pallet jack. He lost his balance causing him to fall off and become trapped between it and a wall. He suffered a broken pelvis and was hospitalized.

An employee was operating a stand-on mower down a hill when the mower started sliding. The employee jumped off the mower and fell to the ground. Their hand was cut by the mower blade, resulting in a laceration and a thumb amputation.

An employee was driving a package car around a building on UPS property when the vehicle struck a bump. The force from hitting the bump unbuckled the employee's seatbelt and caused them to fall out of the vehicle onto the asphalt. The employee was hospitalized and required surgery for fractures to their hip and pelvis.

A television crew was filming in our sanctuary and asked an employee to adjust the temperature in the space. As the employee turned around after adjusting the temperature, he slipped on the lid of an equipment case and fell. The employee twisted when he fell on the floor and injured his right knee below the patella. The employee sustained swelling, a severe hematoma, and pulmonary embolisms requiring hospitalization and surgery.

An employee was preparing meals in a kitchen when an object struck their head, resulting in a laceration to the head and a swollen right eye. The employee was hospitalized and required surgery for a brain injury.

An employee was working from a ladder, changing banners on a wall. The ladder slid backward and gave out and the employee fell, landing on the on the flagstone floor about 13 to 15 feet below. He was hospitalized with a broken elbow and wrist, and he required surgery.

On January 7, 2025, an employee was vacuuming between pews when they tripped over the kneeler in a pew and fell to the floor. The employee sustained a fractured right femur.

An employee was pushing a cart in the parking lot of the rectory when she fell and suffered hip and head injuries that required hospitalization.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
