An employee was walking backwards while installing flashing on a roof. The employee fell approximately 15 feet from the roof and sustained a fractured left elbow and wrist. Fall protection was not worn at the time.

An employee was working on a boom lift when the equipment started to slowly come down and the basket tilted sideways. As the employee tried to push the basket away from the wall his left hand got caught between the top rail of the basket and the wall resulting in fractures to the thumb, index, and middle fingers.
