An employee was performing maintenance on a TCU that was overheating when the TCU blew up. The employee received burns to the face, arm, hands, back of the neck, and hip.

An employee was cleaning leaking plastic from an injection nozzle. As the employee was lifting and dislodging plastic, the nozzle shifted and burst apart. The nozzle struck the employee's mouth and jaw and burned the employee. They also sustained a jaw fracture.

An employee turned on a pump to unload a truck. There was an explosion that caused burns to the employee's hand and face.

An employee fell from the top of a fixed ladder, passing through the ladder's access opening and landing on the floor about 6-7 feet below. He suffered nine broken ribs (four on the left side and five on the right), as well as a punctured lung. He was hospitalized.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
