An employee was submitting samples to the analytical lab. While trying to balance and pick up sample containers, the employee placed their hand on the hinged side of a doorway. The door closed on their finger and partially amputated the fingertip.

An employee was replacing a steel sump cover and the cover flexed upon being released into position, causing the employee's finger to become pinched between the cover and concrete and resulting in a partial finger amputation.
