A forklift operator was setting down a load of corrugated sheet steel when the load shifted. The injured employee attempted to manually stop the load from shifting. The employee's hand was caught under the shifting load, smashing the right middle fingertip and resulting in a fingertip amputation.

A maintenance employee was working on the overhead garage door shaft at the center. He turned the power to the door off so he could work on it. He climbed up a ladder and was working on the door when a customer pulled up and entered their code, and the door started to open. Another employee came to help get the door down and the ladder fell. The maintenance employee fell and sustained a fractured left leg, a broken tooth, and a right shoulder contusion. The employee was hospitalized.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

The injured employee was working in the business lot. A customer was getting into their rental truck and pressed the gas rather than the brake pedal, causing the vehicle to accelerate. The truck struck the injured employee causing a fractured pelvis.

An employee was standing on an 8-foot A-frame ladder leaned against a wall. They fell approximately 4 feet to the concrete floor. The employee was hospitalized with multiple fractures to the left pelvis area and a cut to the right hand and index finger that required five stitches.

An employee was assessing damage to an electrical box while on top of a roll of paper that was held by the clamp arms of a forklift. The employee was pulling wire on the junction box while elevated but came in contact with a live wire and received an electric shock, which caused the employee to fall 20 feet to the ground and sustain a fractured leg, a broken pelvis, and a broken right eye socket.

An employee was cleaning the bathroom in the office of a storage facility. Inhalation of fumes from a cleaner caused respiratory issues and the employee became ill. They were hospitalized.
