An employee was setting up a line for 14-inch PVC pipe production. As the PVC pipe came out of the extruder, it started to sag. The employee lifted the pipe with the left hand to align it and was controlling the remote with the right hand. When the tank came in, the employee's left thumb was caught between the alignment pin and the tank, resulting in a thumb tip amputation between the nail and first knuckle.

On April 4, 2025, an employee was assembling a cooker and was positioning a shaft inside the shell. The shaft was lifted up on each end. When the weight is off the shell, cranes move north or south to position the cooker inside the shell. The cooker shaft suddenly moved and his right middle finger was caught between the cooker shell and a gauge block. The finger was amputated to the bottom of the fingernail.

CLAY AND BAILEY MFG CO

An employee was using a floor-mounted vertical sander to deburr a part. The part got caught between the guard and the sander. The employee's hand was pulled in and the sander crushed the employee's fingers.

An employee was operating a wax injection die. The die's metal lid (which weighed about 125 pounds) fell on his hand. He suffered crush injuries to the index and middle fingers that required surgery.

On June 27, 2024, an employee was assembling a completed mold top weighing 1,261 pounds. The hook from the hoist was under tension. She went to adjust the hook and it moved, catching her left thumb against the mold top. She sustained an amputation to the tip of the thumb.
