On January 9, 2024, an employee was clearing film from a machine roller when they suffered a crushing injury to their right hand and two broken fingers, resulting in an amputation.

On June 28, 2020, an employee was adjusting the chain tension and linkage on a packaging machine. The employee's right hand was crushed in the point of operation of the die.

An employee was changing casings on a clipper machine when the employee's left hand entered the machine's point of operation, resulting in an amputation of the left pinky finger and laceration of the left ring finger.

The employee attempted to manually align boxes by pulling the bottom box forward, causing the top box to slide back and pinch the employee's right middle finger between the two boxes. A chunk of tissue was removed from the finger.

An employee was closing the overhead door on a trailer while standing on a dock. The door started going back up and the employee reached out with his right hand to stop and pull it back down. His right index finger was caught between two panels, amputating his fingertip.

An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.

An employee was using a shaper to cut wood when their right hand contacted the blade, resulting in a partial amputation to the right ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was using a de-clipping machine to cut the metal clips off of sausage sticks. As the employee put a link of sausages into the machine, their hand slipped into the point of operation. A piston pinched the employee's left index finger, causing a partial amputation.

An employee was operating a small tabletop grinder in a food safety quality assurance lab. She was stuffing meat down the grinder's chute when the grinder caught her right hand. Its auger caused an amputation to her middle finger and a partial amputation to her index finger.

An employee was cleaning a part on a pedestal grinder. The employee's middle finger contacted the abrasive wheel, resulting in a partial amputation.

A maintenance employee was removing an inspection plate from an industrial cooker. Hot liquid spilled onto him and burned his hands, arms, and torso area. He was hospitalized.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
