An employee was rolling an 8-inch-by-20-inch piece of metal in a roller. The roller caught his glove and pulled in his right hand. The employee sustained a partial amputation of the right index fingertip.

An employee was bending a piece of 5/8-inch copper tubing on a pipe bender machine. He noticed the machine was not bending the tube properly. As he was trying to correct the problem, his glove became caught, pulling his left index finger into the clamp die of the machine. His fingertip was crushed, resulting in an amputation.
