An employee was bending a piece of 5/8-inch copper tubing on a pipe bender machine. He noticed the machine was not bending the tube properly. As he was trying to correct the problem, his glove became caught, pulling his left index finger into the clamp die of the machine. His fingertip was crushed, resulting in an amputation.

An employee received a call to address the condition of a laser machine. While diagnosing the issue, he saw there was debris that caused the chain to become misaligned. He began realigning the chain when his right ring and little fingers were pinched between the chain and sprocket and amputated.
