An employee was using an electric pallet to move product near the case packer. The employee was struck in the right leg by a forklift that had a clamp attachment. Her leg was pressed between the forklift and the pallet jack resulting in an open fracture to the tibia.

An employee was exiting their vehicle when they slipped and fell on ice, resulting in a lower back injury.

An employee slipped on detergent that had spilled and fell on the floor. The employee landed on their left arm fracturing the radius.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.

The employee fell and their foot became caught in the ladder rungs, causing both the employee and the ladder to fall to the ground. The employee sustained a fracture to the right leg requiring hospitalization and surgery.

An employee was offloading material using a forklift when the hoisted material began shifting. The employee went to manually stabilize the load and their right middle fingertip was pinched and amputated.

An employee was packing candles on a line. They reached down to pick up a 12-pack of candles when their right little finger was caught in the conveyor belt, resulting in an amputation at the distal joint.
