An employee lost her footing while descending a three-step ladder. She fell to the floor about 30 inches below. She suffered a broken left tibia.

An employee was turning in a chair while closing a safe. The safe door closed on his left index finger, causing a soft-tissue amputation to the fingertip.

An employee was walking into the store from the parking lot to begin her shift. Before she made it to the store entrance, a customer walked into her causing her to fall to the ground and sustain injury to her left hip. The employee then walked into the store and fell on the floor. The employee was hospitalized and required surgery.

An employee was helping to set up display tables on the sales floor of a store. The employee tripped over a display table leg and fell onto her back. As she was falling back, she twisted and fractured her ankle.

An employee was moving hand weights to a wall fixture when the shelf gave way and the merchandise struck the employee's head, resulting in a concussion.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was walking to the inventory cage when her ankle gave out and she fell to the floor. The employee sustained a femur fracture and was hospitalized.

An employee was dislodging a pallet that was stuck when boxes of product fell onto them, causing them to fall to the ground. The employee suffered a fractured femur.

An employee was exiting an elevator when he lost balance and fell to the floor, resulting in a fractured hip.

An employee was walking into the stock room when they slipped on a wet floor and fell, resulting in a sprained left wrist and a concussion. The employee was hospitalized.

An employee was climbing down a ladder. She fell from about 3 feet up, grabbed storage racking but could not break her fall, and landed on the floor. She suffered an injury to the left knee, as well as cuts on the right middle and ring fingers. She was hospitalized.

An employee was exiting the building and walking down the stairs with co-workers when she fell down the stairs and landed on her left hip, fracturing it.

An employee was utilizing an inclined conveyor belt in the stock room to transport a bathroom rug to the second floor. She was at the bottom of the conveyor belt when the rug fell into the conveyor roller mechanism, jamming it. She was working to clear the jam when her right arm became caught in the 3.5-inch gap between the powered conveyor belt and the underside roller. The employee was hospitalized with six fractures, including her forearm, index finger, ring finger, wrist, and thumb, as well as a severed artery. The machine was not guarded or locked out/tagged out at the time.

An employee was repositioning merchandise after the floor was stripped and waxed when they slipped on uncured wax and fell to the floor. The employee sustained a fractured right femur.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
