An employee was in a man bucket removing a bridge beam when the wind changed direction. This caused the beam to strike the man bucket and pin the employee in the bucket. They sustained bilateral femur fractures.

Employees were pouring concrete in drill shafts. An overhead power line arced, struck the concrete pump truck, and struck an employee. The employee was shocked and lost consciousness, and was hospitalized to treat right hand/fingertip burns.

An employee was unloading asphalt milling onto a state highway from a dump truck. The dump truck rolled over onto the driver's side and the employee sustained fractured ribs and a punctured lung.

An employee was helping to roll out geotextile material. As the employee was lifting the roll over a wall, it spun and struck the employee's leg fracturing it.

An employee was performing a walk around visual inspection of the crusher recycler when he tripped on a piece of rebar and fell hard on his left hip and forearm. His hip broke and required surgery.

An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.

An employee was using a utility knife to cut honeycomb paper when the knife amputated their left index fingertip.

An employee was cleaning a resident's chimney. While using a drill, the employee's glove became caught in the drill and their finger was fractured.

An employee was grinding some hardfacing on a bulldozer blade when the 9-inch grinder kicked back and lacerated his left leg above the knee. The employee was hospitalized and required surgery.

An employee was operating a rotating tool to place thread protectors on the end of pipe. Her left hand was on the rotating part of the tool when the tool engaged, resulting in a left forearm fracture and a thumb laceration.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
