An employee tripped on a loose piece of strapping and fell to the floor, suffering a broken hip. The employee was hospitalized.

At about 6:00 p.m. on December 21, 2018, an employee was trying to clear a jam in a hopper. The push plate moved while his right hand was in the hopper, resulting in a crushing injury to his right index finger. The finger was surgically amputated.

On September 14, 2018, an employee was checking oil levels in units. While manually rotating a unit's door, the employee's left ring finger was caught between the door and a water coupler, resulting in an amputation.

A bindery operator's finger was caught in a belt and pulley while pointing to the reflector on the line. The tip of his right index finger was amputated.

The injured employee was talking to another employee on the ornament line. She then turned around and tripped over a mail tote. As she fell, she grabbed onto a support pole for the building and sustained a dislocated left shoulder.

An employee's right thumb became caught between two rollers on a coating machine. The thumb was crushed and fractured.

An employee was operating a cutting machine. It jammed. The employee was clearing the jam when the machine's blade amputated the tip of his thumb.

An employee had just finished changing the blade on a bindery machine. He lifted the area where the blade is to inspect it, when he noticed glue on the Allen head. The employee went to scrape it off and the blade partially amputated his right middle finger, and cut his right index, ring, and little fingers. The machine was running at the time.

An employee was loading boxes onto a conveyor when the conveyor stopped. When the conveyor belt restarted, it struck the employee's right hand, resulting in a partial amputation of the right little finger.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.
