He fell backwards and landed flat on his back and his head struck the floor. He sustained a suspected brain bleed and concussion.

The employee had positioned a washer on the lift gate of the delivery truck and was attempting to step around the washer when he fell off the lift gate, which was raised several feet off the ground. As the employee fell, he grabbed the washer and pulled it down on top of him. The washer landed on his abdomen. He was hospitalized with fractured ribs, a fractured C7 vertebra, and a head injury.
