Final narrative
An employee was carrying a roll of shrink wrap. The employee tripped over a pallet and fell, the roll crushing the employee's left little finger against the floor. The employee's fingertip was amputated.
An employee was at the door of a pulpit. His left hand was on the door jamb when the door closed on and pinched his left ring finger, resulting in a left ring fingertip amputation.
An employee was walking over a transfer surface for hot steel bars when the employee fell towards the hot bar. The employee's right leg came in contact with the hot bar, receiving second and third degree burns from the knee down and above the ankle. The employee also received a burn on the left wrist.
An employee was climbing down from the bottom rung of a portable ladder that was leaned against a segment of a caster on a transport car. While dismounting the ladder, the employee lost balance and fell approximately 3' 2" to the floor below. The employee was hospitalized with two fractured vertebrae in the back and required surgery.
On April 16, 2018, an employee was on a 12-foot ladder soldering a water pipe in the ceiling with a propane torch when the torch flashed, causing first degree burns to the employee's right shoulder and face. The employee fell from the ladder and sustained a concussion, fractured ribs, and fractured vertebrae. The employee was hospitalized.
An employee burned 90-foot long scrap rails to cut them in half and make them easier to move. One of the steel rails rolled over, hitting another rail and causing it to also roll over and crush the employee's left ankle. He required surgery.

The employee sustained an amputation to the top knuckle of the middle finger and a fracture to the ring finger. The machine was guarded at the time.
An employee was preparing bundles of 24-foot square metal tubing to be loaded onto a truck and cut the safety band for a bundle. While the employee was removing a piece of dunnage from a bundle, they bumped the bundle with their leg. The metal tubing then fell forward and struck the employee, causing a fractured left ankle.
An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.
An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.
On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
