An employee was advancing a rail to line up for welding with a secondary rail on the weld line when his right middle and ring fingers were caught between the rails. The employee sustained an amputation to the top knuckle of the middle finger and a fracture to the ring finger. The machine was guarded at the time.

An employee was removing a metal shim liner from between stacked manufacturing rolls while a crane was lifting up on a roll to separate them. After the initial lift, the load settled and their right thumb was crushed between the lifted roll and the liner, resulting in amputation at the distal knuckle.

An employee was working at an electric arc furnace. The employee was cleaning ash and shoveling it down a 3-by-3-foot electrode mast hole to the level below. The electrode mast hole did not have an electrode in it due to the furnace being shut down at the time. The employee fell through the hole approximately 20 feet. The employee suffered fractures to their left shoulder, pelvis, L1, L2, and L3 vertebrae; and S1, S2, and S3 sacral bones.
