An employee was walking with a steel pipe. The employee tripped over a portable bandsaw and fell; the sharp end of the pipe struck the employee's hand, amputating about 3/4 inch of the right little finger.

An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

An employee was working on an electrical panel in a mechanical room when an arc flash occurred. The employee suffered burns.

Dept. of Homeland Security - CBP

At about 3:10 p.m. on October 3, 2025, an employee was inspecting a car. Two dogs that had been in another car were leashed and tethered to a bollard. As the employee inspected the first car along with a narcotics detection dog, one of the other dogs came loose and attacked the narcotics dog. The employee was separating the dogs when the other dog bit his left ring finger. The last joint of the finger was injured and part of it was bitten off.

National Air and Space Intelligence Center

An employee was climbing a fixed ladder to perform maintenance in an attic. While unlocking the attic entry, the employee fell from the ladder more than 10 feet to the concrete below. The employee suffered a fractured hip, pelvis, tail bone, arm, and elbow, resulting in hospitalization and surgery.

An employee was on a four-step work stand while sanding an area of an aircraft. He was climbing down the work stand when he tripped over an air hose and fell to the level below, resulting in a fractured femur.

U.S. POSTAL SERVICE

An employee was scanning the placards of pallets of mail bundles when a forklift approached from the opposite side and began lowering and pushing his pallet load forward. It struck the pallet the employee was working on, causing it to shift and strike her. She fell to the ground and sustained a left hip fracture that required surgery.

Boeing (LMTF)

An X-ray machine had lost power and an employee went to replace the power supply. While replacing a cable on the power supply assembly, the employee's right hand contacted the grounding terminals coming out of the back of the power supply assembly and they received an electric shock. The employee was hospitalized.

Ogden Air Logistics Complex

An employee was servicing equipment and had disconnected a 4-inch line. A proportional control air valve on the equipment actuated, releasing a large volume of air at 99 psi into the 4-inch braided stainless steel line hanging down from the valve. The sudden release of pressure caused the line to whip and strike the employee's face. He was hospitalized due to an orbital bone fracture requiring surgery. The equipment was not locked out/tagged out at the time.

United States Postal Service

An employee was performing janitorial duties and carrying bathroom supplies when he slipped on a guardrail mounting post and fell on his side. The employee's hip was fractured and required surgery.
