An employee was throwing plastic trash into a large trash bag when her foot got caught on the bag, causing her to trip and fall. The employee sustained a broken wrist and a mouth injury.

On October 11, 2020, the injured employee was directing a passenger vehicle into the service bay. The vehicle pinned the injured employee against a tire machine. The injured employee sustained a fractured lower left leg requiring hospitalization.

An employee was stepping off a ladder cart when she fell from the last step, landing on the tile/concrete floor and suffering a broken leg and possible broken hip.

The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee was walking to take their break. As they walked from their locker to the entrance of the break lounge, they tripped over a pallet and fell to the ground, resulting in hospitalization due to a concussion.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
