An employee was on top of the conveyor belt junction tower investigating some stoppages of a belt when he noticed some debris on a drum pivoting the belt to the tensioner a few floors below. He used a 5-foot pry bar to try to clear the debris from the area in front of the drum. The pry bar contacted a moving surface and shifted, pinching his right middle finger between the bar and either the electrical conduit or an adjacent machine guard. His fingertip was crushed and avulsed requiring surgical amputation.
An employee was cutting a piece of flat bar stock using an ironworker machine. The machine's hold down jaws caught and amputated the employee's left middle fingertip.
