An employee was installing metal decking on an existing building when the employee tripped over another employee's lanyard and fell off the roof. The employee was hospitalized with a broken right leg, left wrist and ankle, and pelvis. The employee's harness was not tied off at the time of the incident.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
