An employee was using a cordless circular saw to relieve the long edge of a 1/2" rabbit cut on a 1 1/2" piece of chair rail trim. The piece of chair rail kicked out of the employee's hand and his left hand contacted the rotating saw blade. The employee's ring fingertip was amputated and he sustained lacerations to the middle, ring, and little fingers.

An employee was carrying two 52-pound truck batteries when they suffered a back strain.

An employee was working in the fulfillment area carrying large bags of dog food, cat litter, a cube organizer, and book cases. The employee began experiencing abdominal pain and sustained two hernias that resulted in hospitalization.

An employee was carrying a door up an exterior stairwell when he injured his back.

On March 17, 2022, an employee performing a work capacity test was walking with 45 lbs. of weight and began to feel discomfort in his chest resulting in hospitalization.

An employee was disconnecting a wash line. While in a squatting position, the employee was using a rubber hammer to tighten a clamp and sustained a back injury.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
