A maintenance employee climbed a ladder to access an intake on top of a grinder machine and install new equipment. He was attempting to remove a bolt with a wrench when the wrench slipped off the bolt. The momentum of his body caused him to lose his balance and fall 6 feet to the concrete surface below, fracturing his left wrist and left leg.
