An employee was spotting for a skid steer that was loading a bucket into a pickup truck when the employee slipped and fell near the skid steer tracks. The skid steer moved forward and ran over the employee's leg, fracturing the tibia.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.
