Final narrative
An employee was in the back of his trailer offloading a 1500 lb. crate for a customer. There were two straps on the crate holding it to the wall. He released a strap so the customer could unload the crate completely. When he released the strap, the crate tipped over onto him. He sustained a broken shoulder, broken ribs, and an ear contusion.

An employee exited his forklift to hang straps on a trailer and the forklift rolled forward into the trailer, striking his leg. The employee sustained a broken leg and was hospitalized.
An employee was dropping off his trailer after arriving at the terminal. As they were removing a dolly from the rear of the dual trailer set, their hand was crushed in a pinch point between the converter dolly and the back of the lead trailer. The employee sustained fractures to the left hand.
An employee was working on a tractor trailer in the shop. As he was removing a spring pin for the front suspension, the spring unloaded stored energy and caught the employee's right middle finger between it and the mounting bracket for the spring.
