An employee was filling cylinders with propane for forklifts when the nozzle detached from the cylinder. Propane began to spray and the employee attempted to shut off the nozzle. The employee was sprayed with propane and sustained burns.

A driver was carrying out an onsite propane fill from a bobtail truck. There was a fire, and the employee suffered third-degree burns to 36 percent of his body. He was hospitalized.
