An employee was unloading a trailer when a crate fell on his head, causing a concussion.

While on the forklift inside of a trailer, the employee reached through the forks from her seat to place a shipping label on a pallet. The controls were pressed and the pallets came down on her right forearm, resulting in a fracture in two places.
