An employee was operating a forklift to load pallets inside of trailers. While on the forklift inside of a trailer, the employee reached through the forks from her seat to place a shipping label on a pallet. The controls were pressed and the pallets came down on her right forearm, resulting in a fracture in two places.

An employee reached through the mast of a forklift while scanning a bar code on a pallet. The mast was activated and lowered onto her arm, breaking it.

An employee was scanning freight on a forklift through the mast of the lift and bumped the mast lever. The employee's right arm was caught by the mast resulting in a forearm fracture that required surgery.

An employee was unloading a trailer when a crate fell on his head, causing a concussion.

At 7:00 PM on July 6, 2016, at the truck docks, an employee was lifting a plastic 55-gallon drum onto a skid when the drum fell onto his right thumb. His right thumb was crushed, requiring surgery and overnight hospitalization.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
