An employee was trying to remove a manhole cover on a wastewater pit. The cover slipped off a pry bar and caught the employee's finger against the edge of its recess. The employee's fingertip was amputated at the first knuckle.

While walking to her car at the end of the day an employee's left foot slipped off the sidewalk and landed in the parking lot, causing her to lose balance and fall face first onto the sidewalk. The employee sustained a broken nose, facial fracture, concussion, and contusions.

An employee was walking from a parking lot to a walkway. The employee slipped on the icy curb/walkway and fell to the ground, suffering a broken fibula and left elbow.

An employee was on the table of a machine. She went to jump down from the table and fractured her lower leg on the ground. The employee was hospitalized.

An employee was loading a flatbed trailer when a forklift bumped into the trailer. A wheelset began to roll in his direction and the employee jumped off the trailer to avoid it. The wheelset caught him on the leg and he landed on his shoulder on the ground. He sustained a laceration below the left kneecap and a dislocated shoulder.

An employee jumped off the loading dock to assist a package delivery driver who fell. He sustained an ankle fracture that required surgery.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
